I, the undersigned, understand and acknowledge that drinking alcohol at the MB Healthcare holiday party/event carries potential risks and dangers beyond the control of MB Healthcare Services LLC and any related facilities or entities . By attending the MB Healthcare Services LLC holiday party/event, I hereby assume all liability for any form of injury or damage that occurs to me due to the consumption of alcohol by myself or any event participant, and I hereby agree to release and hold harmless MB Healthcare Services LLC and any related facilities or entities and officers, agents, or employees, from all liability to me, my next of kin, my conservators, assigns, heirs, guardians or other legal representatives for any and all claims, demands, losses or damages, suits, fines, including court costs and attorneys' fees, for any injury, death, or damaged property arising out of making available/serving alcohol at this event. I hereby waive all legal rights to pursue any form of legal action against MB Healthcare Services LLC and any related facilities, their officers, agents, or employees. Additionally, I understand that by attending the holiday party/event, I am exposing myself to individuals who may or may not be vaccinated against COVID-19. By signing below, I acknowledge the contagious and evolving nature of COVID-19 and voluntarily assume the risk that I may be exposed to COVID-19 at the holiday party/event and become infected as a result. I understand that becoming infected may impact me, my family, and my personal contacts. I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any expense, liability, illness, injury, disability, or death related to contracting COVID-19 while attending the holiday party/event hosted by MB Healthcare Services LLC. I hereby release, covenant not to sue, discharge, and hold harmless MB Healthcare Services LLC and any related facilities or entities from all claims of any kind arising out of COVID-19. I understand and agree that release of liability includes any claims towards MB Healthcare Services LLC and any related facilities or entities, its officers, agents, or employees, whether a COVID-19 infection occurrence exists before, during, or after attendance at the holiday party/event.
